CONSENT AND RELEASE

I agree to participate in the audition/interview process for a new one-hour primetime game show for Recorder Television Network, currently intended for possible production and initial broadcast in 2009-2010.

The audition/interview process may include one or more of the following:

(i) short informal interview on camera,
(ii) participation in a fun trivia quiz/game composed of questions
(iii) participation in further auditions/interviews.

Auditions will either take place at the AAJ TV premises.

Upon selection, a series of on-camera interviews will be conducted on family life which can be on or off location.

Each participant will be selected based on his/her range of  knowledge, personality and his/her enthusiasm to participate on the game show.

I hereby irrevocably consent to the recording and use of my voice, actions, likeness, name, sobriquet,picture, photograph, silhouette, appearance, and other reproductions of my physical likeness (as the same may appear in any still camera photograph and/or motion picture film or tape) and biographical
information (i.e., collectively “likeness”) in connection with the audition/interview process.
